Step 2 - Structured Home Evaluation
Room-by-room, hazard-focused, clinically guided.
A Senterra-trained evaluator walks through your home using our Structured Home Evaluation checklist — covering entries, stairs, bathrooms, kitchens, bedrooms, lighting, flooring, and more.
Every potential hazard is documented with location, severity, and context, so nothing is missed. -
Step 3 - Senterra Score™ & Priority Plan
Your home’s findings are converted into a Senterra Score — a clear 0 to 100 rating that reflects your overall safety profile. Along with the score, you receive a clinically prioritized plan that organizes every recommendation into three stages: Stabilize (urgent items to address first), Strengthen (mid-term improvements that support safety and mobility), and Enhance (long-term or design-forward upgrades). The result is a simple, medically guided roadmap tailored to your home’s needs.

You can act on your plan with your own contractor, or we can connect you with certified installers in the Senterra network.
