With combined backgrounds in clinical care, physical therapy, and operational strategy, we saw a clear pattern: most home hazards that lead to falls are predictable and preventable, yet families rarely receive the guidance they need to address them with confidence.

Every evaluation follows the Senterra Field Assessment, a structured, room-by-room walkthrough that identifies risks with clinical precision. Findings are translated into a Senterra Score, giving families a clear, objective measure of their home’s safety profile. From there, we create a prioritized safety plan organized into three stages—Stabilize, Strengthen, and Enhance—so you know exactly what to address now, what to improve next, and what will support long-term independence.
